  • 2.Fees Challan Fees Paid (xerox copy)
  • 3.School Leaving Certificate / Transfer Certificate (xerox copy)
  • 4.PG Teacher Joining form
  • 5.NEET – PG Rank Card (xerox copy)
  • 6.10th and 12th Std. Marksheet (xerox copy)
  • 7.1st MBBS to 3rd MBBS Marksheets (xerox copy)
  • 8.University Attempt Certificate (xerox copy)
  • 9.Provisional Degree Certificate (xerox copy)
  • 10.Certificate of Provisional Registration (xerox copy)
  • 11.Character Certificate (xerox copy)
  • 12.Certificate of Registration (xerox copy)
  • 13.Degree Certificate (xerox copy)
  • 14.Internship Completion Certificate (xerox copy)
  • 15.Aadhar Card (xerox copy)
  • 16.Cast / EWS Certificate (xerox copy)

Forms

Post Graduates joining the Institute are requested to submit the following forms along with the above documents to the ‘Students Section’:

Hostel admission

Kindly submit the Hostel admission form for Interns (🔗) and Post Graduate Students (🔗), as the case may be to the respective hostel rector.

Hostel fees of interns and post graduate students is deducted from the stipend.
